  • Transitioning from out of network (including concierge) to using insurance to cover surgery or other procedures is something we’re happy to help you with.

    To try and get a partial refund for the cash pay visits you’ve had with us, we provide a document called a “superbill”, and you submit that document and a claim to your insurance company. Claim forms for each insurance can be found here

    For surgery, once that decision is made, we submit for insurance approval in the usual fashion subject to deducibles and out of pocket maxes. Any previous visits may not count against your deducible until approved by your insurance company.

  • These packages are most appropriate for patients seeking a longer episode of care, but for whom surgery is probably not in the near future. Depending on length, these packages allow us to take a more holistic assessment of the root cause of an injury. These packages have longer visits, preferred scheduling including visits outside standard hours, and telehealth options.

    Three Month:

    • Best for a single, focused problem of the upper extremity

    • Allows us to develop a comprehensive diagnosis, and start the treatment pathway

    Six Month:

    • Best for patients who want to evaluate and treat several problems, or who want to begin a more comprehensive assessment of the root causes of their pathology

    • Includes regenerative treatments, when appropriate

    • Preferred scheduling and hours

    Twelve Month:

    • Comprehensive musculoskeletal care, including bone health, biomarkers, hormone and peptide optimization, and definitive management of injuries

    • Includes regenerative treatments, when appropriate

    • Preferred scheduling and hours, including telehealth
